Using a concept model

Take the following scenario and describe the journey being taken using terms from the draft version of ISO 13940:2026 – a system of concepts for the continuity of care.

Alex, a 21 year old student, has an accident while playing quidditch for their university.

The first-aiders on site call 999 to ask for an ambulance.

The team dispatch an ambulance crew.

The ambulance crew arrive and take charge.

They assess Alex’ condition and determine that their lower arm bone is broken.

They transport Alex to the nearest hospital and handover Alex’ care to the hospital emergency department.

The emergency department ask the X-Ray department to take an image of the lower arm.

Alex goes for the X-Ray and returns.

The X-Ray department send the image across to the Emergency department, who determine that Alex should attend the fracture clinic.

They transfer Alex to the fracture clinic with the X-Ray, the report from the ambulance crew and the assumed diagnosis – a fractured lower arm.

The fracture clinic confirm the diagnosis, treat Alex and provide advice on how to manage the arm at home or University.

Alex will need help with dressing from members of their household while their arm is mending.

An appointment is scheduled for five weeks time so that Alex’ arm can be reviewed.
